Millcreek provides an array of community based services and supports under the Home and Community Based Services Waiver for Individuals with Mental Retardation and other Developmental Disabilities. Administered by the Mississippi Department of Mental Health (DMH), services and supports funded by Medicaid are provided to eligible individuals and their families to enable the individuals with disabilities to remain with their families, in their community, or return to their community from a congregate care setting.
Services and supports include:
- Attendant care
- Respite care
- Day habilitation
- Pre-vocational
- Supported employment
- Behavior support/intervention
- Therapy services

| Day Habilitation Services provide an avenue for individuals to attain daily living, social, communication, self-help, and other adaptive skill outcomes to enhance their quality of life. Activities are planned based on individual needs, preferences and interests. For instance, the individual might need frequent rest periods, small activity groups, or a special diet; services are tailored to fit the needs and desires of each individual. This requires that each individual complete a habilitation planning process as described in the BMR Record Guide. Formal Person-Centered Planning enhances the planning process by incorporating diverse viewpoints. |

| The Prevocational Services program develops habilitation plans and activities directed toward skill development in community settings and center-based, non-work activities. Activities in prevocational services are designed to teach habilitative goals rather than job-specific skills. The best place to learn job-specific skills is on a job in the community. Our Plan of Care is designed to accomplish a quality-of-life that is similar to that of any other citizen who would like to work in the community-at-large. Millcreek advocates for an individualized community job for all individuals in our pre-vocational programs. Since a forty-hour week of competitive employment is often not feasible for people with significant disabilities, Millcreek Community Services will utilize pre-vocational services as an adjunct to Support Employment. The more minimum wage employment a person with significant disabilities receives the less Pre-Vocational services will be utilized. The term Pre-Vocational is then true to its original meaning and persons with significant disabilities are not left in segregated Shelter Workshop environments all their lives. |

| Millcreek Community Services’ philosophy of providing a continuum of care in the least restrictive environment possible can be seen clearly in its efforts to integrate individuals into competitive employment. Minimum standards for employment services will be met through procedures outlined in the Support Employment Services ID/DD Waiver. |

| Physical, occupational, speech/language/hearing therapy is
provided by licensed staff based on assessed needs on the individual’s Plan of
Care. Services may be delivered in a
variety of locations depending on the needs of the individual. |

| Our qualified staff provides assistance in daily living skills and support involvement in community activities. Activities of daily living, i.e., household services, meal planning and preparation, assisting with self-administration of medications, and accessing community resources i.e. shopping, appointments, recreation, and other related activities are all examples of support an attendant care staff can offer. |

Qualified and licensed staff provide support services in an effort to relieve the family members or caretakers in meeting planned or emergency needs. Respite care can be provided in a variety of settings, including the family home and community locations. Categories of respite care include: In-Home Companion Respite, In-Home Nursing Respite, and Community Respite. |

| Licensed or certified staff provide services to assess
problem behaviors which are disruptive to the community, school, work, or home
environments and design intervention strategies to support the individual
successfully in that environment. |
