Day Habilitation |

Day Habilitation Services provide an avenue for individuals to attain daily living, social, communication, self-help, and other adaptive skill outcomes to enhance their quality of life. Activities are planned based on individual needs, preferences and interests. For instance, the individual might need frequent rest periods, small activity groups, or a special diet; services are tailored to fit the needs and desires of each individual. This requires that each individual complete a habilitation planning process as described in the BMR Record Guide. Formal Person-Centered Planning enhances the planning process by incorporating diverse viewpoints.

Millcreek CBS is committed to community inclusion, and will provide weekly opportunities to interact with people and environments in the community. These outings will be done in groups of no more than three individuals. All participants will be given a schedule for daily personal development in community settings typically used by persons without disabilities. Goals and objectives will be set using an Ecological system of teaching, which requires personal development to occur in the most natural setting available.
Through the assessment process, activities will be selected for the individual’s daily/weekly schedules in order to promote the following:

- - Community Participation
- - Dignity
- - Choice
- - Supporting Contributions
- - Safety
The overall goal of the program is to support the individual so that he/she can participate in everyday activities with as much independence as possible, enhancing the participant’s quality of life.
Millcreek CBS will provide a mid-morning snack, a noon meal, and afternoon snack. Individuals are offered choices about what they eat.
Staff to individuals being served ratio is at a minimum two staff to each seven individuals receiving services. Only licensed nurses provide medical services, including the administration of medication. Millcreek CBS employs physical, occupational, speech and behavior therapists, and incorporates therapist training into daily activities.
While in the centers, individuals are served in clean, pleasant buildings with a minimum of 50 square feet per individual being served.
Millcreek CBS can accommodate a wide range of needs (e.g., ambulatory, verbal people to non-ambulatory, non-verbal people who may need suctioning and/or be fed through a G-tube, etc.). Eligible individuals will not be denied enrollment in the program of their choice based on the severity of their disability.
